Release of Information

As a client of Sweetser, a confidential record is kept by your health care team. This record contains health care history, details of your treatment, and the progress you have made during your treatment. The information is confidential and will be released only as described in Sweetser's Notice of Privacy Practices. 

Release of Records

If you would like a copy of your medical record as it pertains to treatment received through Sweetser, please print and complete the Release of Information Form and mail or fax it to the address below.

We cannot accept e-mail requests for copies of confidential records as an original signature is required from the client or their authorized representative.
