Summary

About the Position:This position is responsible for performing all new client comprehensive assessments and referring these clients to any and all appropriate Sweetser services.This is a Monday through Friday per diem position providing back up to manage volume of assessments.The Ideal Candidate Would:·         Complete client assessments as scheduled by the Promise Line.·         Responsible for all assessment documentation to include: Diagnosis, assessment, Mental Status, Level of Care, Progress notes, releases etc within the Electronic Client Record. ·         Submits referrals for immediate scheduling within identified standards with designated service(s)·         Identify improvements in all assessment processes.·         Proficient with Electronic Client Record.·         Ability to perform concurrent documentation

Education Requirements

·         Master’s Degree in a related field (Psychology, Human Development, Social Work, etc)·         LCSW,  LCPC, LMFT 

General Requirements

·         Must maintain a valid state driver's license.·         Experience with assessment of adults, children and residential.·         Experience with assessment of adults, children and residential.·         Knowledge of Medications, crisis assessment skills and familiarity with community resources. ·         Customer service skills·         Excellent communication skills·        Computer Skills
