Assertive Community Treatment (ACT) is an intensive community based (outpatient) treatment program with a multi-disciplinary team of providers that supports eligible adults and older teens (aged 16+) who have a diagnosis of schizophrenia, schizoaffective disorder, or another type of severe and persistent mental illness. The goal of the ACT Program is work with the client to identify goals that will help improve the client’s functioning while remaining in the community.
Working with the ACT team, you will have access to a clinician, substance use clinician, intentional peer support, registered nurse, medication manager, and case management/ employment specialist.
This is a community-based total team approach who work with you to promote independence, rehabilitation and recovery.
