Adult Community Integration

The primary goal of Sweetser’s community-based adult services is to partner with adults experiencing a serious mental illness to assist them in their journey toward recovery. It is Sweetser’s core belief that treatment for mental health and co-occurring disorders should be provided in the home and community.

Adult Community Integration (ACI) provides adult consumers with a designated community support professional who works collaboratively with them to identify and secure the support and treatment necessary to achieve their recovery plan goals while remaining in the community of his or her choice.

How can a Community Integration Worker help?

Adults can be assisted in developing a crisis prevention program; with making appropriate and safe decisions during times of heightened emotional stress; partnering with various service providers such as outpatient therapy, medication management, continuing education; and work placement.

Typical issues we can assist with:

  • Housing
  • Difficult behavior in the community
  • Emotional instability
  • Need for integration of care, including medical, emotional, social, and vocational

Eligibility

An adult is eligible for community integration if they meet the MaineCare eligibility requirements for Section 17. Continuation of services must be approved every three months by the administration service organization contracted by the State of Maine to oversee all MaineCare services.
