High Fidelity Wraparound is an intensive care coordination process that brings together the people and resources important in a young person’s life—including family members, providers, schools, community partners, peer supports, and other trusted individuals. Together, the team develops one individualized plan based on the youth and family’s vision for success.
A Family-Driven Approach
Unlike traditional case management, High Fidelity Wraparound is guided by the youth and family’s strengths, needs, voice, and choice. The family serves as the primary decision-maker, while a dedicated wraparound coordinator, family and youth peer supports, professionals, and natural supports work together as one team.
The process looks beyond a diagnosis to understand underlying needs across every area of a young person’s life, including home, school, health, and community involvement. Family and youth peer support are central to the model, giving participants an opportunity to learn from individuals who have navigated similar challenges.
Connecting Care and Supporting Transitions
High Fidelity Wraparound strengthens Maine’s children’s behavioral health system by connecting and coordinating existing services—not replacing them.
Whether a youth is receiving outpatient treatment, in-home support, crisis services or hospital care, or transitioning from a higher level of care back to the community, the wraparound team helps the family navigate changes, maintain continuity, and work toward shared goals.
The program seeks to prevent avoidable crises and out-of-home placements whenever possible while helping youth remain safely at home, succeed in school, and stay connected to their communities.
Success means families feel heard, empowered, and supported. It means improved well-being, greater family confidence, fewer disruptions, and a lasting network of support that continues after formal services conclude.
